Creating a consent form in the Form builder

You can use Mahmee to request and document consent from patients. Here’s how:

  1. In Form Builder, create your Form settings, Start page, and Finish Page.

(MAHMEE TIP: Do not check “Hide this form from the patient” since you would like your patient to provide consent on the form.)

2. Add a Section header (optional). To add section headers, select the question type 'Title.' Enter the section header in the 'Question Text' field. Section headers appear in bold, purple text, centered in on the screen with a section break line underneath.

3. Select the Add + button in the 'Question List.'

4. Enter consent or acknowledgement form language in 'Question Text'. Text entered in this box is black, bold, and centered.

5. Select 'Consent Checkbox' for Question Type.

6. Under the Question Type, enter consent or acknowledgement language for the patient to click. For example: "I have read and understand the cancellation policies and appointment policies."

7. Check the 'Required' box.

8. Select Publish or 'Save as Draft.'
