Adding documents or files to a patient’s medical record

Upload files like documents, superbills, .pdfs, or images to a patient’s medical record or to a patient’s appointment session

Providers can upload files like documents, superbills, .pdfs, or images to a patient's medical record or to a patient's appointment session.

Settings can be marked so that the file can be visible or hidden to the patient and/or members on the patient's Care Team.

Adding files or documents to a patient's medical record

  1. From the patient's medical record, select the kebab icon (three dots) on the top right corner of the screen.

  2. Select 'Documents' -> 'Upload.'

  3. Enter the document or file's name and description.

  4. Use the dropdown menus to choose the visibility setting of the file.

  5. Click 'Select a file' and upload your document.

  6. Select 'Upload'.

  7. To edit, download, or delete a document or file, select the three dots on the right.


Adding documents, superbills, or files to a patient's appointment session:

  1. From the patient's appointment session details page, select 'Upload' from the Documents section.

  2. Chose the file or document, or superbill to upload.

  3. Give the file or document a name, description, and set the visibility settings.

  4. Select Upload.

  5. The document or file can now be accessed via the appointment session details or the patient's medical record.


Locating documents or files on a patient's medical record

  1. From the patient's medical record, select the overflow menu (three dots) on the top right of the screen.

  2. Select 'Documents'.

  3. Documents (such as PDFs, notes, videos, or images) added to the patient’s account, sent via messages, or uploaded to appointment sessions, can be found here.

  4. You may search for existing documents using the search bar and even filter your search results.

  5. Select the document or file's name to view.


Editing, deleting, or downloading documents from a medical record

  1. From the patient's medical record, select the overflow menu (three dots) on the top right of the screen.

  2. Select 'Documents'

  3. Select the kebab icon (three dots) to the right of the document to be edited or deleted.

  4. Select either edit, delete, or download.

After a document has been deleted, you can 'restore' it to the patient's medical record. You will need to confirm that you have permissions to do this from your organization's account admin.
