Add a pregnancy, baby or loss to a patient's record

Update the patient's medical record with a birth, loss, or pregnancy

Patients and providers can update the patient's medical record to include a pregnancy, baby, or loss.

To add a pregnancy to a patient's medical record:

  1. Navigate to the patient's medical record.

  2. Under Medical Record, scroll down to the Pregnancy & Baby section.

  3. Click the +Add button.

  4. Click Add Pregnancy.

  5. Enter pregnancy information and click Save at the bottom of the page.

  6. The patient’s medical record will then update with the entered pregnancy information.


To report a birth/add a baby to a patient’s record:

  1. Navigate to the patient’s profile.

  2. On the Medical Record tab, scroll down to Pregnancy & Baby.

  3. Click Report a Birth next to the patient's current pregnancy.

  4. Complete the questions to add information about the baby.

  5. Click Save at the bottom of the page.

  6. If the birthing person had multiple babies, click on Save & Add Another Baby

  7. The birth information is now viewable on the patient’s profile and the Patient’s Dashboard


To record a loss on a patient's medical record

If a patient has an existing pregnancy listed on their profile, you are able to report a loss.

  1. Navigate to the patient's medical record.

  2. On the Medical Record tab, scroll down to Pregnancy and Baby.

  3. Select Report a Birth, then select Report a Loss

  4. Record the outcome of the pregnancy

  5. The patient’s medical record will be updated and the pregnancy information will be removed from the patient’s dashboard.
