Export data from surveys, screenings, appointments, and forms.

With the Data Export add-on feature, organizations can download information captured through your Forms, Appointment chart fields, Interactions, and Screenings for reporting, billing, and/or grant applications.

Data exports result in a downloadable .csv (spreadsheet) file. After downloading the .csv file, you may edit or filter data results to meet your specific needs.

Exporting data from forms


When creating a form, you may select which questions will be included in a data export. For each question in the form, select if you'd like the question response to be part of your data reporting. Then, create a column title for the .csv file. For example: If you ask patients, "What is you is your estimated due date?," you may want to label the column, "Due date."

After you have created the form, it's time to assign the form to patients. Then, responses to your form are available to export.

  1. From the 'Manage Resources', select 'Forms'.

  2. Select the dot icon to the right of the form and click 'Export CSV'.

  3. The .csv file will populate and download to your device.

Exporting data from Appointment Charts

When creating an appointment chart, you may select which data is included in the data export. For each question in the appointment chart, select if you'd like the question response to be part of your data reporting. Then, create a column title for the .csv file.

After you have created the appointment chart, it's time to begin using it during appointments. Then, responses to questions in the appointment chart are available to export.

  1. From the 'Manage Resources' dropdown, select 'Appointment Charts'.

  2. Select the dot icon to the right of the appointment chart and click 'Export CSV'.

  3. The .csv file will populate and download to your device.

Exporting data from Surveys and Screenings

Responses to answers in a screening or survey are available for export. Each question in the screening or survey will be included in your export.

  1. From your provider dashboard, select Surveys and Screenings from the purple navigation bar on the left.

  2. Select the dot icon on the top right corner of the page. Click 'Export Questionnaire to CSV.'

  3. Select the 'Export' next to the Survey or Screening.

Exporting Data from Interactions

Data fields included in an Interactions data report columns include;

  • Recorded Date/Time

  • Start and End Time

  • Start and End Date

  • Patient name and UUID

  • Author's Name

  • Type of Interaction

  • What was written in the Interaction

  • If the Interaction was marked viewable to a practitioner

  • If the Interaction was marked viewable to the patient

To export Interaction data;

  1. From the organization's dropdown, select 'Manage Resources.'

  2. Select Data Export, 'Export Interactions.'

3. Select the timeframe you want to export from.

4. Select 'Export'


Exporting patient data

Patient data exports retrieve data from all patients in your organization. Data report columns include;

  • Patient Name, UUID, and Date of patient's birth

  • Pregnancies and Live births experienced

  • Medical conditions listed

  • If pregnant, the expected due date

  • If postpartum, the date of baby's birth

  • Date the patient was entered into Mahmee

  • Patient zip code

*This export capability is only activated for some organizations and account administrators.*

To export patient data;

  1. From the organization's dropdown, select 'Manage Resources.'

  2. Select Data Export, 'Export Patient data.'

3. Confirm your selection

4. Select 'Export'
